Stormwater Pollution Prevention Plan

Answer all the following questions. All fields are required, unless otherwise indicated.

Threatened or Endangered Species

Are there any Federal or State listed rare, threatened, and endangered species and/or designated critical habitats that are located in the project area? How do I determine if such species are located in my project area?

Are there specific requirements in your Erosion and Sediment Control Plan (E&SC) or Stormwater Pollution Prevention Plan (SWPPP)?

Will my project require a Stormwater Pollution Prevention Plan?

Answer questions A-E below. All questions must be answered.

If you answer yes to any of the questions, your project WILL require an SWPPP prior to submittal of the Notice of Intent. See Part III.F of the permit.

If you answer no to all the questions, your project WILL NOT require an SWPPP to be submitted to MDE.

A. Chemical Additives or Polymers

Will your project involve the use chemical additives or polymers for sediment control?

Yes, I plan to use chemical additives or polymers for sediment control.

Check each box below to indicate that you have read and understand each of the documents listed.

Do you plan on using a Cationic Polymer?

If you have you been approved for the use of a Cationic Polymer, provide the date of the approval.

(MM/DD/YYYY)

No, my project will not involve the use chemical additives or polymers for sediment control.

B. Prohibited Discharges and Discharge Limitations

Will construction activity include disturbing material or soils with known contamination by toxic or hazardous substances (Part I.D.6: Prohibited Discharges and Part III.B.3: Discharge Limitations for Sites Discharging to Impaired Waters), dewatering requiring turbidity benchmarks (Part III.B.4), or has the Department notified you with a request to install additional controls to meet water quality standards (Part III.B.1: General Effluent Limitation to Meet Applicable Water Quality Standards)?

Yes, construction activity has the potential for disturbing material or soils with known contamination by toxic or hazardous substances.

Check each box below if applicable to your project.

Does the construction activity include disturbing material or soils with known contamination by toxic or hazardous substances (e.g. from sites managed under Maryland’s Voluntary Cleanup Program (VCP) or Land Restoration Program (LRP))? See Part III.B.3 [PCBs] and Part I.D.6 [PROHIBITION] of the 20-CP.

The Turbidity Benchmark Monitoring Data Reporting Form can be found here: Appendix D 20-CP: Turbidity Monitoring Report Form and Instructions.

No, construction activity will not disturb material or soils with known contamination by toxic or hazardous substances.

C. Pollution Prevention Requirements

Will you be implementing controls associated with any of the pollution prevention activities referenced in Part III.A.3: Pollution Prevention Requirements? (exception for sites under 5 acres with ONLY concrete washout, building materials storage/handling, fertilizer use to establish vegetation, and NO other activities listed under Part III.A.3)

Yes, I plan on implementing controls associated with the activities requiring pollution prevention measures, referenced in the permit.

Select from the list below the activities that require additional pollution prevention measures.

No, I do not plan on implementing controls associated with the activities requiring pollution prevention measures, referenced in the permit.

D. Sharing Permit-Related Functions

Will your project involve the sharing of permit-related functions between and among operators on the same site?

My project is within a Common Plan of Development or Sale.

Is this NOI for an existing common plan of development?
